Responsibilities
วางระบบบริหารยาและความพร้อมด้านกฎหมาย
- ออกแบบโครงสร้างระบบจัดเก็บยา, ระบบเบิกจ่าย, และระบบตรวจนับยาให้สอดคล้องตามกฎหมาย อย.
- จัดทำคู่มือ WI การจัดการยา (รับเข้า–จ่ายออก–คืน–ทำลาย)
- ออกแบบผังงานห้องยา/เวชภัณฑ์, ตั้งมาตรฐานรับ–เก็บ–จ่าย–ทำลาย, จัดทำเอกสารขออนุญาต
ควบคุมการจ่ายยาออกฤทธิ์สำคัญและความปลอดภัยการใช้ยา
- จัดทำทะเบียนคุม (บัตรประจำตัว/บัญชีรับ–จ่าย)
- ทบทวนใบสั่งยา, DUR/MUE, Medication Reconciliation, Label
สนับสนุนทีมแพทย์และพยาบาลในการรักษา (Clinical Support)
- สนับสนุนข้อมูลยา ขนาดยา และอาการไม่พึงประสงค์ในผู้ป่วยดีท็อกซ์และจิตเวช
- เข้าร่วมทีมสหวิชาชีพ (Multidisciplinary Team) เพื่อประเมินผลการรักษาและการใช้ยา
- จัดอบรมเจ้าหน้าที่เกี่ยวกับการใช้ยาอย่างสมเหตุสมผล
ออกแบบระบบจ่ายยาและเชื่อมต่อ HIS (Dispensing & Integration System)
- ออกแบบ workflow การจ่ายยาแบบอัตโนมัติหรือกึ่งอัตโนมัติ
- ประสานงานกับทีม IT เพื่อเชื่อมต่อข้อมูลยาในระบบ HIS
- ตรวจสอบความถูกต้องของข้อมูลยาและผู้ป่วยในระบบแบบ real-time
เตรียมเอกสารมาตรฐาน HA / AACI (Accreditation
- จัดทำหลักฐานตามเกณฑ์ HA ด้านการบริหารยา, การควบคุมสารเสพติด, และความปลอดภัยของผู้ป่วย
- เข้าร่วมการตรวจประเมินภายในและภายนอก (Internal & External Audit)
- จัดทำรายงานข้อเสนอแนะเพื่อพัฒนาระบบยาอย่างต่อเนื่อง
บริหารคงคลัง
- วางแผนจัดซื้อกับจัดซื้อ
Qualifications
- ปริญญาตรีเภสัชศาสตร์ (B.Pharm/Pharm.D) และมีใบอนุญาตประกอบวิชาชีพเภสัชกรรมที่ยังไม่หมดอายุ
- อายุ 30 ปีขึ้นไป
- ประสบการณ์งานเภสัชกรรมโรงพยาบาล/คลินิก ≥ 3–5 ปี
- เคยมีบทบาทวางระบบ/สตาร์ทอัพโรงพยาบาล หรือผ่านงานเตรียมรับรอง HA/GPP จะพิจารณาเป็นพิเศษ
- มีประสบการณ์จัดการยาออกฤทธิ์/จิตเวช
- จริยธรรมวิชาชีพสูง ละเอียดรอบคอบ รักษาความลับข้อมูล (PDPA)
- พร้อมปฏิบัติงานเป็นกะ/On-call **ตามความจำเป็น และเดินทางสนับสนุนสาขาได้
- สุขภาพพร้อมปฏิบัติงาน, ไม่มีประวัติเกี่ยวข้องคดียาเสพติด
- สามารถสื่อสารภาษาอังกฤษได้
